An investigation has begun into the case of a German doctor who accidentally gave a patient a lethal overdose while providing out-of-hours cover on his first shift in the UK.
Dr Daniel Ubani gave David Gray 10 times too much painkiller while working for a Cambridgeshire health trust.
He was given a nine-month suspended prison sentence in Germany for negligence which caused Mr Gray's death.
The charity, Action against Medical Accidents, has been advising David Gray's family. Its chief executive Peter Walsh wants a wider review into out-of-hours NHS care.
