Five years ago the medical profession was rocked by revelations that babies operated on at Bristol Royal Infirmary had died unnecessarily.

Two consultant paediatric surgeons and the hospitals Chief Executive were struck off the medical register and an exhaustive Inquiry into the Bristol tragedy is just about to be published.

But could Bristol happen again? Panorama investigates the efforts of the medical profession to tackle the problems that the tragedy brought to light and considers whether surgeons should have their results made public.
