Coroners under the spotlight
Graveyard
Review will examine the way deaths are investigated
When the Shipman inquiry resumes in the autumn, it will move into a new phase of examining the systems which failed to spot the former GP's trail of murder.

Dame Janet Smith's inquiry has close connections with another review which is looking at the work of coroners, and is likely to lead to much closer monitoring of the way in which all deaths are investigated. The review may also recommend reductions to the number of post mortem examinations and inquests.

Tom Luce, the man heading the review, has given an interview to the World At One ahead of his ideas being published at the end of the month.

Listen to Jane Dreaper's report.
