The son of a man who died after being given a lethal dose of painkiller by a doctor flown in from Germany to provide weekend cover has called for vetting procedures to be tightened.

Dr Stuart Gray, whose father David died in February 2008 after accidentally receiving 10 times the the maximum dose of diamorphine from Dr Daniel Ubani, told the programme that without a change to the current out-of-hours system, "there will be other cases".

His comments follow the publication of an interim report into out-of-hours GP care by the Care Quality Commission (CQC), prompted by Mr Gray's death, which found primary care trusts may be failing to spot patient safety issues because of a lack of scrutiny.

Health minister Mike O'Brien said that he welcomed the CQC's "robust" findings, which he said, ended "the cosy reassurance you sometimes had in some of these reports".

Dr Ubani was given a nine-month suspended sentence and fined £4,500 by a court in Germany for causing death by negligence.
