England have made an official complaint to the International Cricket Council over an umpire decision in the fourth Test against South Africa. Third umpire Darryl Harper failed to turn up the volume on his television, when assessing England's referral for a caught behind, ending Graeme Smith's run. Jonathan Agnew reports from Johannesburg.
