West Mercia Police have apologised to the family of a man shot dead at a village post office in Worcestershire after an inquiry found the force guilty of "clear failings".

The Independent Police Complaints Commission (IPCC) found that detectives made no effort to arrest Anselm Ribera before he shot Craig Hodson-Walker last year.

IPCC commissioner Len Jackson told the Today programme that there was "no doubt" individual detectives were to blame, adding that Craig Hodson-Walker's family were "failed both by the system itself and by individual officers".

Assistant chief constable Simon Chesterman agreed that the original investigation "should have been better" and that "basic policing went wrong".
