A former head of the NHS has warned that proposed changes to the system are focussing on the wrong areas - calling for a shift in healthcare towards treating those with long-term conditions.

Explaining his concerns to John Humphys, Lord Crisp said that most of the patients in the NHS were now those with from long-term conditions, which needed a different kind of care - focused less in big hospitals and more at home.

"We have a health service that was designed for the old problems of the 20th century, and we need a health service for the 21st century," he said.

"This government needs to change its tack. It needs to get people agreeing about the problem and then working out what are the best solutions."
