A report out today offers new proposals for how to deal with care for those who cannot look after themselves.

Geoff Bostock who cares for his wife with dementia told the Today programme's Zubeida Malik that he feels he is being penalised under the current system.

"In the back of my mind is the constant feeling of uncertainty," he said.

Sir Merrick Cockell, chairman of the Local Government Association, told the Today programme that there is no time to "tinker around" and "radical change" needs to be looked at.

"This is an important step forward, and we must build on that," he told John Humphrys.
