Sandra Banks said she was given the wrong treatment by Dr Daniel Ubani shortly before he treated David Gray in Cambridgeshire last year.
An investigation has been launched after Dr Ubani accidentally administered 10 times too much painkiller to Mr Gray, 70, while working on call for a health trust.
Mr Gray subsequently died.
Dr Ubani was given a nine-month suspended sentence in Germany for negligence over the death of Mr Gray, 70, in 2008.
