Your list seems complete, Nicholas. Dr Jozef Venglos was Aston Villa's manager in 1990-91, but that was before the Premier League began in 1992.

Because they are deemed to be playing the ball when it is with "playing distance", players are allowed to shield it. I am told the word "obstruction" no longer appears in the Laws of the Game, and has been replaced by "impeding."
